To be eligible for this program, simply download the form, fill it out, and mail, email, fax or drop it off at the Customer Information Center at the Downtown Transit Center (DTC).
What if the facility I’m traveling to is not listed?

One Personal Care Attendant is allowed. The cost is $4 per trip on the Medivan and free for all other services.
Can I take advantage of this program if I require special medical treatment during transportation?
Since this program does not employ emergency vehicles, we are not able to accommodate passengers requiring special medical treatments during transportation.
What should I do if I am unable to keep my appointment or no longer need the trip?
Please cancel your appointment as soon as you know you no longer need the trip so that someone else can use your seat. You can call (209) 943-1111 to cancel no later than 12:00 p.m. the business day before the scheduled trip.
